Referral letter from GP or other doctor
Medicare card, DVA card, pension card
Private Medical Insurance information if applicable
Reports, X-rays, MRI’s, scans and any other relevant information
At your first consultation Dr Matthews will conduct a thorough assessment of your history and an examination to determine the most appropriate diagnoses and treatment options available.
